HOPE Project – Homeless Services & Mobile Health Clinic

Homeless individuals can receive services through the HOPE,  (Homeless Outreach for People Empowerment) Project.  The Hope Program is limited to serving the chronically homeless.  The HUD definition of chronically homeless is:

  • Permanently disabled, having a condition impairing their ability to function
  • Homeless at least a year or more
  • Sleeping outside or in a car – cannot be couch surfing or in a motel

The HOPE Project Mobile Health Clinic, our partnership with Tri-City Health Center, provides health and social services to people in southern and eastern Alameda County experiencing homelessness. Click here for a schedule of its weekly stops.

  • Monday, 9:00 AM -Noon , 460 N. Livermore Avenue, Livermore (showers, clothes, breakfast, bag lunch
  • Tuesday & Thursday, 3:30 PM -6 PM, 4360 Central Avenue, Fremont (shower and dinner)
  • Wednesday, 7:30 AM-9:30 AM, 4181 Irvington Avenue, Fremont (breakfast 7-9 a.m.)
  • Thursday, 9:00 AM- 11:00 AM, 24362 Thomas Avenue, Hayward (a hot breakfast is
    served at 8 a.m.)
